
Dr. Loris has launched a program of comprehensive care for varicose vein patients. She had extensive vascular surgery experience during her residency in Milwaukee and has taken formal hands on training from Dr. Gregory Spitz, a national expert in varicose vein disease.
The major symptoms caused by varicose veins are fatigue, heaviness, aching, burning, cramping, inflammation and even ulceration of the lower legs.
The care of varicose veins involves thorough diagnostics to assess the deep and superficial venous systems as well as the perforators. Therapy is then directed at correction of the problem.
For reflux at the saphenofemoral junction, we perform saphenous vein stripping. For the larger, tortous subcutaneous varicosities, Dr. Loris performs Transilluminated Powered Phlebectomy. This procedure involves instillation of a tumessence fluid, which contains lidocaine and epinephrine, into the subcutaneous tissue. This fluid assists the dissection and also minimizes postoperative pain and bleeding. A light source is introduced through one small incision to view the superficial varicose veins. Through another 5 mm incision the powered phlebectomy device is advanced and used to remove the visualized vein. After the procedure the patient is wrapped in an occlusive dressing and the incisions are left open so blood and fluid are allowed to drain out, minimizing hematoma formation.
The patients are fully ambulatory after the procedure. Clinical studies suggest that this method enables the surgeon to remove veins using a minimal number of small incisions and short operative times, which results in an easier recovery for the patient and good cosmetic results.
