Ventral Hernia

The concept of “sports hernia” is relatively new. The definition is chronic groin pain in athletes greater than 3 months with a normal exam for inguinal hernia and in the absence of other etiologies for the pain.

Several authors have reported laparoscopic preperitoneal mesh repair for this diagnosis. In two studies polypropylene mesh was used, while one used a biologic mesh. The authors reported finding inguinal hernias at the time of the operation up to 50% of the time despite negative preoperative workups. After mesh repair, 88% to 95% of patients were pain free and active in sports after 4-8 weeks of recovery.

Groin pain resolved equally between patients diagnosed with a hernia on laparoscopy and those without. Recent evidence suggests that heavy weight polypropylene can harden and shrink over time, thus we feel an alternative mesh should be used in athletes. Surgesis ™ has shown to benefit these patients, but may break down over time and allow recurrent herniation.

We prefer Parietex 4 x 6 inch anatomical polyester mesh, whose hydrophilic properties resist shrinkage and hardening. We are preparing a prospective study assessing laparoscopic preperitoneal mesh repair of sports hernias using Parietex mesh.
